A parent or guardian must meet the following criteria:

The majority of child care services are covered. Funding is not available for child care provided by a family member who lives with you, licensed occasional care, or recreational care.

You have to perform the following tasks:
- For you, your spouse (if applicable), and your children, provide your name, birth date, and gender.
- For each family member, provide digital photographs of one of the following government-issued identity documents:
